It used to be a standard and favourite breakfast in middle-class households in Kerala, although I'm not sure it's so common these days. I sort of grew up on it (now I am in my early 60s). We called it 'walla-choru' (water-rice). We used to have it with roasted (not fried) pappadam, coconut chutney or dried-fish chutney. And yes, like you mentioned, buttermilk is a matter of choice - we loved it 🙂
An interesting trivia: I remember hearing that WHO had declared it as one of the most nutritious breakfasts in the world - the fermentation of the rice converts quite a bit of the carbs/starch into proteins, and so you have a well-balanced breakfast, that gets used up quickly while farming or any other physical activity. It obviously is not for the desk-bound workers that we are today! 🙂
